The participant understands that there is a risk of personal injury in the course of instruction Specifically, participant agrees to hold harmless the school and all other individuals, organisations, sponsors promoters, operators, hosts, instructors, associations, schools, owners, officials, directors, employees and other participants connected with the gym/event from all losses, damages, injuries, causes of actions, claims or complaints in the event that the participant is damaged or injured in any way during the participation, instruction and/or performance of any exercise during any activity associated with the gym/event location or during transit to or from the gym/event.
Because of the physical demands of martial arts and/or personal defence instruction, participant understands that he/she must be in good physical condition to participate in any classes or events.
Participant understands that in case of injury the only medical treatment SHADOWBOX SUNSHINE COAST will provide is First Aid. Participant agrees that any photos, videos, audio recordings taken of him/her in connection with the seminar, class, event or promotion can be used for publication, promotion, articles, shows and adverts without additional permission nor compensation at this time or at any time.
Participant states:
- I am not under the influence of any drugs, alcohol or any other intoxicant, and will not be under the influence of these substances at any of the sessions or classes I attend
- I am voluntanty participating in these activities entrely at my own risk
- I agree to strictly obey instructors and observe safety rules.
- I have disclosed any disabilities I may have to the coaches at Shadowbox Sunshine Coast
- I am not suffering from any undisclosed illnesses
- I am over 18 years of age (if not over 18 years of age parent or guardian must sign)
- I have read and understood this waiver and release of lability and agree to its provisions.
